We ask you not to include health information, your Medicare number or your Social Security number in that message, and we do not request them.
Consent records. When you check the permission-to-contact box on our form, we record the date and time, the consent language displayed to you, and the technical details of the submission. We keep this record to document your consent.
Information collected automatically. Our website may collect standard technical information such as your browser type, device type, the pages you view and the time of your visit. We use this only to understand how the site is used and to keep it working properly.
What we do not use. We do not use advertising pixels, ad-network tracking tags, or third-party marketing trackers on this website.
